Free therapist toolkit
Simple printable worksheets you can use with your clients, free. Print one sheet or all of them, and adapt them to the way you work.
Please read before you use these sheets. They are general, everyday worksheets written by Therapist Circle. They are not validated questionnaires or outcome measures, they do not diagnose anything, and they are not a substitute for your own assessment, risk assessment or clinical judgement. Use them only within your own training, competence and professional body’s guidance, and keep completed sheets as you would any client record. If a client is at risk, follow your usual safeguarding and risk procedures.
What’s in the toolkit
You may print, copy and share these sheets with your clients for free, including in paid sessions. Please keep the small Therapist Circle line at the bottom of each sheet and do not sell the sheets themselves. To print all four, use your browser’s own Print option.
Weekly mood diary
Each day, give your mood a number from 0 (very low) to 10 (very good), and jot down anything that seemed to affect it. Short notes are fine. Bring it to your next session.

Session preparation sheet
A few minutes before your session, note anything you want to bring. You don’t have to fill in every line.
Since we last met, the main thing on my mind has been…
Something that went better than I expected…
Something I found hard…
One thing I’d like to talk about today…
A question I have for you…
Anything I’d like to do differently between sessions…

Values card sort
Cut out the cards, or tick them on the sheet. Sort them into three piles: very important to me, quite important, and less important right now. Then choose up to five from the first pile and talk about what each one means to you. Add your own on the blank cards.
